CBD has also been shown to have neuroprotective properties in cell cultures as well as in animal models of several neurodegenerative diseases, including Alzheimer’s,xii,xiii,xiv stroke,xv glutamate toxicity,xvi multiple sclerosis (MS),xvii Parkinson’s disease,xviii and neurodegeneration caused by alcohol abuse.xix Nabiximols (trade name Sativex), which contains THC and CBD in roughly equal proportions, has been approved throughout most of Europe and in a number of other countries for the treatment of spasticity associated with MS. It has not been approved in the United States, but clinical trials are ongoing, and two recent studies reported that nabiximols reduced the severity of spasticity in MS patients.xx,xxi There have been limited clinical trials to assess the potential efficacy of CBD for the other indications highlighted; however, a recent small double-blind trial in patients with Parkinson’s disease found the CBD improved quality-of-life scores.xxii

However, according to a research report published by Dr. Ethan Russo, Director of Research for the International Cannabis and Cannabinoids Institute, terpenoids produce an “entourage effect”.
At sufficient doses (400-600 mg), CBD can alleviate situational anxiety, such as public speaking. Interestingly, cannabis cultivars, or strains, that are high in CBD and low in THC are better than other cultivars for alleviating depression. But when used over a long period of time, any kind of cannabis could make depression worse. Although clinical trials in people haven’t yet been completed, there is very compelling “petri dish” evidence that CBD can reduce inflammation, for painful conditions such as Crohn’s disease, and fight some cancerous tumors. There is a massive amount of scientific research being done on CBD right now, and we are likely to see many medical breakthroughs in the next decade.
In addition to fighting inflammation in the body, CBD oil may reduce anxiety by directly affecting the brain. Studies have found that CBD actually lowers activity in the amygdala and increases prefrontal cortex activation, two parts of the brain involved in anxiety. There is also evidence that CBD is able to activate hippocampus neurogenesis, aka regenerate new neurons! This activates CB1 receptors, which has a positive balancing impact on GABA and glutamate levels, associated with reducing anxiety.
Dry mouth: As is the case with many other hemp- and marijuana-based products, CBD oil often leads to a condition known as dry mouth (or cottonmouth). This is likely due to cannabinoids altering receptors in the lower jaw that trigger salivation. In most cases, mild discomfort and stronger-than-average thirst are the only issues associated with dry mouth.
There is no scientific evidence or research on CBD as an effective treatment for migraine—in large part because it has not been formally studied. However, it may still be a viable topical option for some patients with joint and muscle pain associated with migraine. “If you have a lot of neck pain or soreness, it is perfectly reasonable to use CBD oil. It may even prevent nausea and vomiting,” Dr. Silberstein says.

The case study notes that advanced chemotherapeutic agents had failed to control the blast counts (cells in the blood and bone marrow) in the patient and had devastating side effects that ultimately resulted in death. The cannabinoid therapy, on the other hand, had no toxic side effects and only psychosomatic properties, with an increase in the patient’s vitality.
Evidence from animal studies have begun to characterize the details of how CBD acts in the brain, and human studies of patients with and without anxiety disorders are starting to validate CBD’s efficacy as an anti-anxiety treatment. While there are more unknowns than knowns at this point, Grant says he doesn’t discount all the anecdotal CBD reports. “You hear somebody say, ‘Hey, I gave this to myself and my kid and we feel a lot better,’ and we should never dismiss that kind of information,” he says. He points out that many modern medicines were discovered when researchers followed up on exactly this sort of human trial-and-error evidence. “But we still need to do the studies that confirm whether all the good things are true, and how much to give, and how to give it,” he says. “These are all questions that need to be answered.”
Understanding CBD’s analgesic, or pain-relieving, interactions with the ECS can shed light on CBD’s other interactions and effects. Importantly, the ECS participates in our bodies’ pain processing, but when CBD is introduced to our ECS, it stops the body from absorbing a pain-regulating compound known as anandamide — one of our body’s’ own natural cannabinoid molecules. There has actually been a substantial amount of experiments done on cannabidiol that has proved its potential healing powers. Dr. Sean McAllister, a scientist from the Pacific Medical Center in San Francisco, made an astounding discovery about CBD. McAllister has been studying cannabinoid compounds for 10 years now in search of new therapeutic interventions for various cancers. He discovered that cannabidiol is a potent inhibitor of cancer cell proliferation, metastasis, and tumor growth.

In Canada, hash oil – defined as a chemically concentrated extract having up to 90% THC potency – was approved for commerce in October 2018. In the United States, regulations specifically for hash oil have not been issued as of 2019, but hemp seed oil – along with hulled hemp seeds and hemp seed protein – were approved as generally recognized as safe (GRAS) in December 2018, indicating that "these products can be legally marketed in human foods for these uses without food additive approval, provided they comply with all other requirements and do not make disease treatment claims".
The existence of substantial clinical investigations regarding CBD has been made public. For example, two such substantial clinical investigations include GW Pharmaceuticals’ investigations regarding Sativex and Epidiolex. The United States Federal Government does not define ‘hemp’ exactly, but they do define ‘industrial hemp’ to be any part of a cannabis plant, whether growing or not, that is used solely for industrial purposes (fiber and seed) with a THC concentration of no more than 0.3 percent when dried. In contrast, Hemp.com defines it as “the fiber and seed part of the Cannabis Sativa L. plant, opposed to the flower part of the plant which is ‘legally considered’ marijuana.”
In April 2018, a Food and Drug Administration advisory panel unanimously recommended approval of Epidiolex (cannabidiol oral solution) for the treatment of two rare forms of epilepsy - Lennox-Gastaut syndrome and Dravet syndrome, which are among the two most difficult types of epilepsy to treat [18, 16]. Epidiolex was granted Orphan Drug designation as well as Fast Track Approval from the FDA for further study in these hard to treat conditions. Notably, phase 3 clinical trials of Epidiolex have demonstrated clinically significant improvement in Lennox-Gastaut syndrome and Dravet syndrome . On June 25th, 2018, Epidiolex was approved by the FDA to be the first CBD-based product available on the US market.
Cannabinoids can be agonists, inverse agonists or inhibitors. The agonists simply stimulate a bodily function once they adhere to their respective receptors. Inverse agonists associate themselves with the same receptors as agonists, while causing a chemical reaction opposite to the ones caused by agonists. Inhibitors simply stop a chemical reaction or response once bound to their receptors.
