The primary psychoactive component of Cannabis, delta 9-tetrahydrocannabinol (Δ9-THC), demonstrates its effects through weak partial agonist activity at Cannabinoid-1 (CB1R) and Cannabinoid-2 (CB2R) receptors. This activity results in the well-known effects of smoking cannabis such as increased appetite, reduced pain, and changes in emotional and cognitive processes. In contrast to THC's weak agonist activity, CBD has been shown to act as a negative allosteric modulator of the cannabinoid CB1 receptor, the most abundant G-Protein Coupled Receptor (GPCR) in the body [5]. Allosteric regulation is achieved through the modulation of receptor activity on a functionally distinct site from the agonist or antagonist binding site which is clinically significant as direct agonists (such as THC) are limited by their psychomimetic effects such as changes to mood, memory, and anxiety[5].
What exactly is cannabidiol (CBD) and more importantly, what does it do? Those questions and more are at the heart of this comprehensive guide to one of the most fascinating and important compounds of the cannabis plant. Cannabis plants are chemical powerhouses that produce more than 400 different compounds. Not all of those compounds are unique to marijuana, of course, and appear in many other species of plants. That’s why marijuana can smell like pine trees or taste like fresh lemons. But of those 400 compounds, more than 60 of them are totally specific to the plant genus Cannabis. Scientists call these special compounds “cannabinoids.” However, not all cannabinoids are created equal. The American Medical Association believed that evidence of cannabis’ harmful effects was limited and the act would prevent further research into its medicinal worth.

However, Bonn-Miller told Live Science that he thinks cannabis research is on the upswing. "If we flash forward five years I think you'll see more studies," he said. Those studies could reveal more conditions that CBD may be helpful for and may also reveal that some of the reasons why people say they use CBD oil are not supported by the science but are instead a placebo effect. "And that's why we need to do the studies," he said.

To be fair, the paucity of data about CBD’s efficacy and safety in part reflects the federal government’s irrational restrictions on cannabis research. Because cannabis is classified as a Schedule 1 drug, you need a license from the Drug Enforcement Administration to research it and, until two years ago, you could use only the cannabis grown at the University of Mississippi.
Dr. Silberstein advises against obtaining a product in states in which CBD oil is not currently legal or regulated. Illegal forms of CBD oil could be spiked with artificial THC which could be very harmful to patients. The hard truth about most anxiety disorders is that they make you bummed out, even in your downtime. Especially in your downtime. CBD, when combined with THC, has antidepressant properties, which regulate your emotional response and your general way of thinking. There are two ways for that to happen: When ingested, CBD activates 5-HT1A receptors. By doing so, it enables the brain to send more serotonin signals, thus reducing the anxiety and boosting your general mood.
Unrefined hemp oil does not have a very long shelf life. The oil quickly goes rancid, unless it is stored in dark containers in a refrigerated environment. People who use unrefined hemp oil generally purchase it in small amounts so that it will not become rancid. The oil is also not suitable for cooking, because it has a very low smoking point. Refined hemp oil is much more shelf stable, although many of the benefits of it are not present after refining.
Unfortunately due to the disappointing and down right inaccurate position of the federal government in classifying Cannabis as a schedule one drug, most research institutions risk federal funding if they conduct real research on Cannabis. This has dramatically limited the potential for real research by real scientists to be conducted. Most human studies of CBD have been done on people who have seizures, and the FDA recently approved the first CBD-based drug, Epidiolex, for rare forms of epilepsy. Clinical trials for other conditions are promising, but tiny. In one Brazilian study published in 2011 of people with generalized social anxiety disorder, for example, taking a 600-mg dose of CBD (higher than a typical dose from a tincture) lessened discomfort more than a placebo, but only a dozen people were given the pill.
To make matters more confusing, nine states (including California, Washington, and Colorado) let residents buy cannabis-based products with or without THC. Nearly two dozen other “medical marijuana states” allow the sale of cannabis, including capsules, tinctures, and other items containing CBD or THC, at licensed dispensaries to people whose doctors have certified that they have an approved condition (the list varies by state but includes chronic pain, PTSD, cancer, autism, Crohn’s disease, and multiple sclerosis). Sixteen more states legalized CBD for certain diseases. But because all these products are illegal according to the federal government, cannabis advocates are cautious. “By and large, the federal government is looking the other way,” says Paul Armentano, deputy director of the Washington, DC–based National Organization for the Reform of Marijuana Laws (NORML), but until federal laws are changed, “this administration or a future one could crack down on people who produce, manufacture, or use CBD, and the law would be on its side.”

The United States Federal Government does not define ‘hemp’ exactly, but they do define ‘industrial hemp’ to be any part of a cannabis plant, whether growing or not, that is used solely for industrial purposes (fiber and seed) with a THC concentration of no more than 0.3 percent when dried. In contrast, Hemp.com defines it as “the fiber and seed part of the Cannabis Sativa L. plant, opposed to the flower part of the plant which is ‘legally considered’ marijuana.”
Early preclinical findings also suggest that CBD may have therapeutic value as a treatment of substance use disorders. CBD reduced the rewarding effects of morphinexxxviii and reduced cue-induced heroin seekingxxxix in animal models. A few small clinical trials have examined CBD and/or nabiximols (THC/CBD) for the treatment of substance use disorders; however, the available data are not sufficient to draw conclusions. NIDA is supporting multiple ongoing clinical trials in this area.
I am 81 years old next month. I have been in serious pain from Fibromyalgia since I was in my 50s. Also for the last 5 or 6 years, I have suffered from painful arthritis in my shoulders, back, neck and knees. I walk with a walker and have to sit down after doing any chores that take standing for more then 8 or 10 minutes. My care-giver told me about Hemp oil for pain so I decided to try it. That being said, it was unlikely that the federal government was interested in pursuing individuals complying with state-mandated regulations surrounding legalized cannabis for recreational use, although the CSA law still gives them authority to do so. However, the new Trump administration may change this thinking and users of legal marijuana and legal dispensaries await further action and clarifying rules.

An animal study using mice found repeated administration of CBD may help the hippocampus regenerate neurons, which could be useful for treating anxiety or depression. Research shows both SSRIs and CBD may promote neurogenesis. This is significant, because evidence suggests that severely impaired neuronal plasticity may influence suicidal behavior. Future research comparing CBD and SSRIs effect on neurogenesis could open up promising new avenues in how we understand depression and how to most effectively treat it.
