Epidiolex is the first FDA-approved treatment in the U.S. that contains a purified drug substance derived from marijuana -- CBD -- and the first treatment for Dravet syndrome. In September 2018 the FDA rescheduled cannabidiol from a C-I controlled substance to a C-V controlled substance, meaning it has a proven medical use but a low risk of abuse. This change allows Epidiolex to be marketed in the U.S.

Hemp oil is also rich in "super" polyunsaturated fatty acids, most notably gamma-linolenic acid and stearidonic acid. Although these are not essential fatty acids, they may help reduce the symptoms of atopic dermatitis and other skin conditions. However, the amount of these non-essential fatty acids varies according to the quality of the hemp plant the acids were derived from.

A 2016 literature review indicated that cannabidiol was under basic research to identify its possible neurological effects,[11] although as of 2016, there was limited high-quality evidence for such effects in people.[21][95][96] A 2018 meta-analysis compared the potential therapeutic properties of "purified CBD" with full-plant, CBD-rich cannabis extracts with regard to treating refractory (treatment-resistant) epilepsy, noting several differences.[97] The daily average dose of people using full-plant extracts was more than four times lower than of those using purified CBD, a possible entourage effect of CBD interacting with THC.[97]


I recently was a guest at a medical marijuana educational event that highlighted the work of researcher Michael Backes. During his presentation he made a statement about CBD that I have never heard anywhere else that CBD is “regulating” (my word) the effects of THC. I asked the Nurse Practitioner at the event, Ivy Lou Hibbitt of Certicann.com, what he meant by that and she said it was her understanding of Michael’s comment that he takes CBD to reduce the psychoactive effects of THC. Has this property of CBD, that it can lessen psychoactive effects, ever been researched elsewhere?

“DEA will continue to support sound and scientific research that promotes legitimate therapeutic uses for FDA-approved constituent components of cannabis, consistent with federal law,” acting DEA administrator Uttam Dhillon said in a press release. “DEA is committed to continuing to work with our federal partners to seek ways to make the process for research more efficient and effective.”
That doesn’t mean it’s completely risk-free, but the likely risks are more of the allergic reaction variety, Romanowski notes. “It is a component of an essential oil, and reactions are not frequent, but common in a small percentage of the population,” he says. “People should do a small patch test on their skin before generally applying the product over their body.”
Based on reviews, smoking or vaporizing CBD vape oil seems to have less effects when compared to other methods of administering CBD, such as tinctures, capsules and sprays. These cannabinoid-rich extracts can pose risks to patients who consume them. The exact composition of different available oils is frequently unknown. They are not checked for quality by external certified laboratories for the presence of residual solvents, or contaminants such as microbes, pesticides, heavy metals or mycotoxins. The lack of standardisation of both the cannabis starting material and oils makes it impossible to fully evaluate their therapeutic effects over time and, hence, their medicinal value.
More than 60 cannabinoids not found in any other plant to date have been identified in cannabis. The most common ones are cannabigerol (CBG), cannabichromene (CBC), cannabidiol (CBD), Δ9-THC and cannabinol (CBN). The cannabinoid distribution varies between different cannabis strains and usually only three or four cannabinoids in concentrations above 0.1% can be found in a single plant.
A. The FDA is aware that several states have either passed laws that remove state restrictions on the medical use of marijuana and its derivatives or are considering doing so. It is important to conduct medical research into the safety and effectiveness of marijuana products through adequate and well-controlled clinical trials. We welcome the opportunity to talk with states who are considering support for medical research of marijuana and its derivatives to provide information on Federal and scientific standards.
A review published in 2017 in the journal Frontiers in Pharmacology described how CBD may work to protect the hippocampus — the part of the brain responsible for several important functions, such as learning, memory and navigation — during times of stress, and may also help prevent brain-cell destruction that results from schizophrenia. Another 2017 review published in the journal Annals of Palliative Medicine summarized a handful of studies that suggest cannabis oils containing THC or CBD, or both, may help with chronic pain management, but the mechanism is unclear.
Understanding CBD’s analgesic, or pain-relieving, interactions with the ECS can shed light on CBD’s other interactions and effects. Importantly, the ECS participates in our bodies’ pain processing, but when CBD is introduced to our ECS, it stops the body from absorbing a pain-regulating compound known as anandamide — one of our body’s’ own natural cannabinoid molecules. Inhibiting the absorption of this compound shunts excess quantities into the bloodstream that in turn reduces pain. One study has revealed that CBD targets alpha-3 (α3) glycine receptors to suppress chronic pain and inflammation associated with dysfunctional glycine receptors, which are an important target for pain processing in the spine. In both humans and animal models, CBD has been shown to have a variety of anti-inflammatory properties.
In addition to acting on the brain, CBD influences many body processes. That’s due to the endocannabinoid system (ECS), which was discovered in the 1990s, after scientists started investigating why pot produces a high. Although much less well-known than the cardiovascular, reproductive, and respiratory systems, the ECS is critical. “The ECS helps us eat, sleep, relax, forget what we don’t need to remember, and protect our bodies from harm,” Marcu says. There are more ECS receptors in the brain than there are for opioids or serotonin, plus others in the intestines, liver, pancreas, ovaries, bone cells, and elsewhere.
A. Conducting clinical research using marijuana involves interactions with several federal agencies. This includes: a registration administered by the Drug Enforcement Administration (DEA); obtaining the marijuana for research from the National Institute on Drug Abuse (NIDA), within the National Institutes of Health, or another DEA-registered source; and review by the FDA of an investigational new drug (IND) application and research protocol. In general, the human body has specific sites that are allotted to cannabinoids, called the cannabinoid receptor sites. Tetrahydrocannabinol (THC) and cannabidiol (CBD) are two types of cannabinoids found naturally in the resin of the marijuana plant, both of which interact with the cannabinoid receptors that are found throughout the body. Although THC and CBD have been the most studied cannabinoids, there are many others identified to date including cannabinol (CBN), cannabigerol (CBG), Cannabidivarin (CBDV), and Tetrahydrocannabivarin (THCV) that can be found within the medical cannabis [10]. The cost of treatment varies: Depending on the dispensary and the dosage, it can range from around $100 a month to more than $1,000. Despite the cost, which is not covered by insurance, CBD medicines are drawing great interest for children with severe, intractable epilepsy. California and Colorado, which were among the first states to legalize medical marijuana, have become hot spots for such patients. Before other states legalized medicinal CBD use, some families moved to these states so they could have access to the compound.

Multiple sclerosis (MS). There is inconsistent evidence on the effectiveness of cannabidiol for symptoms of multiple sclerosis. Some early research suggests that using a cannabidiol spray under the tongue might improve pain and muscle tightness in people with MS. However, it does not appear to improve muscle spasms, tiredness, bladder control, the ability to move around, or well-being and quality of life.
Serotonin: Most people associate serotonin with happiness. However, the role of this neurotransmitter is very complex, and its effects depend on where it is and what it binds to. (Contrary to popular belief, more serotonin is not necessarily better — dysfunction can stem from low or high levels of serotonin, as well as from malfunctions with their receptors.) Serotonin has at least 14 different receptors, but CBD specifically binds to 5-HT1A which is thought to have the strongest role in anxiety disorders. The anti-anxiety drug buspirone also binds to this particular receptor, which explains the anti-anxiety effects of CBD on rats exposed to stressful situations.

CBD is currently available in Canada within a 1:1 formulation with tetrahydrocannbinol (THC) (as the formulation known as "nabiximols") as the brand name product Sativex.
