Schizophrenia. Research on the use of cannabidiol for psychotic symptoms in people with schizophrenia is conflicting. Some early research suggests that taking cannabidiol four times daily for 4 weeks improves psychotic symptoms and might be as effective as the antipsychotic medication amisulpride. However, other early research suggests that taking cannabidiol for 14 days is not beneficial. The conflicting results might be related to the cannabidiol dose used and duration of treatment.

Along with its better-known counterpart, THC (delta-9-tetrahydrocannabinol, the chemical that produces the marijuana high), CBD is one of more than 400 compounds found in the oils of cannabis plant species, which include marijuana and hemp. Unlike THC, CBD will not make you high. Unrefined hemp oil does not have a very long shelf life. The oil quickly goes rancid, unless it is stored in dark containers in a refrigerated environment. People who use unrefined hemp oil generally purchase it in small amounts so that it will not become rancid. The oil is also not suitable for cooking, because it has a very low smoking point. Refined hemp oil is much more shelf stable, although many of the benefits of it are not present after refining.

"It's important to know that the research in this area is in its infancy, partly because we haven't really understood much about CBD until relatively recently," said Marcel Bonn-Miller, an adjunct assistant professor at the University of Pennsylvania Perelman School of Medicine. He pointed out that the classification of marijuana as a Schedule 1 drug by the DEA makes it difficult to get material to use in laboratory studies. Schedule 1 drugs have a high potential for abuse, according to the DEA, and are illegal under federal law.

From a pharmacological perspective, Cannabis' (and CBD's) diverse receptor profile explains its potential application for such a wide variety of medical conditions. Cannabis contains more than 400 different chemical compounds, of which 61 are considered cannabinoids, a class of compounds that act upon endogenous cannabinoid receptors of the body [11]. Cannabinoid receptors are utilized endogenously by the body through the endocannabinoid system, which includes a group of lipid proteins, enzymes, and receptors that are involved in many physiological processes. Through its modulation of neurotransmitter release, the endocannabinoid system regulates cognition, pain sensation, appetite, memory, sleep, immune function, and mood among many other bodily systems. These effects are largely mediated through two members of the G-protein coupled receptor family, cannabinoid receptors 1 and 2 (CB1 and CB2)[12, 8]. CB1 receptors are found in both the central and peripheral nervous systems, with the majority of receptors localized to the hippocampus and amygdala of the brain. Physiological effects of using cannabis make sense in the context of its receptor activity as the hippocampus and amygdala are primarily involved with regulation of memory, fear, and emotion. In contrast, CB2 receptors are mainly found peripherally in immune cells, lymphoid tissue, and peripheral nerve terminals [9].

A number of studies over the last two decades or more have reported that CBD has anti-seizure activity, reducing the severity of seizures in animal models.vi,vii In addition, there have been a number of case studies and anecdotal reports suggesting that CBD may be effective in treating children with drug-resistant epilepsy.viii,ix,x However, there have only been a few small randomized clinical trials examining the efficacy of CBD as a treatment for epilepsy; the total number of subjects enrolled in these studies was 48. Three of the four studies reported positive results, including decreased frequency of seizures. However, the studies suffered from significant design flaws, including failure to fully quantify baseline seizure frequency, inadequate statistical analysis, and a lack of sufficient detail to adequately evaluate and interpret the findings.viii Therefore, the currently available information is insufficient to draw firm conclusions regarding the efficacy of CBD as a treatment for epilepsy; a recent Cochrane review concluded, there is a need for “a series of properly designed, high quality, and adequately powered trials.”xi

For this study, 24 people with social anxiety disorder received either 600 milligrams (mg) of CBD or a placebo an hour and a half before performing a simulated public speaking test. Additionally, 12 other people with social anxiety disorder performed the same test without receiving any CBD treatment. Results revealed that pre-treatment with CBD significantly reduced anxiety, cognitive impairment, and discomfort while participants were delivering their speech.
